Peroneal Artery Bypass for Limb Salvage

Abstract
Background:
@EN Peroneal artery is known to be inferior run-off vessel to the other infrapopliteal arteries due to the absence of direct communication with pedal arch and the difficulty in approach. But peroneal artery would be the only run-off vessel in
some
cases
and some author reported the good result of peroneal artery bypass graft.
@ES Purpose:
@EN To ascertain the value of peroneal artery bypass for limb salvage, we analyzed the result of peroneal artery bypasses conducted on 35 patients.
@ES Method:
@EN We performed 37 peroneal artery bypasses on 35 patients from Jan 1987 to May 1995 at SNUH. The mean age of the patient was 40.0 year and the median follow-up duration was 16 months(range: 1 mo~100 mos). All but two were male. The etiologic
disease
was TAO(26) and ASO(11). Most of the graft materials were autogenous vein(in situ vein: 16, non reversed translocated vein: 6, reversed vein: 10) and the others were composite grafts(5). The primary indication was for limb salvage in 33
patients(35
operations). Patency rate was obtained by Kaplan-Meier method.
@ES Result:
@EN There were 5 postoperative graft occlusions and 4 of them resulted in graft failure. There was no other complications. There were total of 10 graft failures and 7 of them occurred within one year after operation. Limb salvage rate at 5 year
was
97%(one below knee amputation). The primary and secodary patency rates were 77.9% and 80.9% at 1 year, 69.4% and 2.9% at 2 year, 69.% and 66.8% at 4 year, and 69.4% and 66.8% at 5 year, respectively. There was no significant relationship between
graft
patency and inflow site, graft type, and etiology.
@ES Conclusion:
@EN Peroneal artery bypass is a recommendable procedure with an acceptable limb salvage rate.
